Özet
Introduction: The aim of the study was to evaluate local control and survival in patients with non-small cell lung cancer (NSCLC) who underwent stereotactic body radiotherapy (SBRT) using the lung-optimized treatment (LOT) module with the CyberKnife M6 (CK-M6).
Methods: 49 lesions in 46 patients were treated between May 2019 and April 2025. A planning 4D-CT was obtained in a free-breathing modality. Tracking modality was selected based on the target's visibility on the device. The median prescribed dose was 50 Gy in 5 fractions (30- 55 Gy in 1-7 fractions). Survival analysis was performed using Kaplan-Meier and Cox regression analysis for 49 lesions. A p-value < 0.005 was considered significant.
Results: The median follow-up was 30 months (5-80). The median age was 68 (46-81), and 46% (20/43) of the cases were adenocarcinomas. Three patients diagnosed with radiologically. Median lesion size was 20 mm (8- 57). Zero-View, 1-view, and 2-view tracking were applied to 21, 22, and 6 lesions, respectively. SBRT was applied to 47 primary lesions and 2 lung metastases originating from lung cancer. Grade 3 and higher toxicity was nor observed in any case. Twoyear local recurrence-free survival (LRFS), overall survival, and progression-free survival rates were 81,7%, 71,7%, and 65,2%, respectively. In multivariable analysis, 12-month partial response (HR= 15,67, 95% CI: 3,53- 69,47; p <0,001), beam-on time ≤ 22 min (HR= 4,85, 95% CI: 1,59- 14,82; p=0,005), and dose <50 Gy (HR= 3,08, 95% CI: 1,04- 9,13; p=0,042) were unfavorable factors for OS. In terms of PFS, female gender (HR= 8,14, 95% CI: 1,36- 48,56; p=0,021) and 12- month partial response (HR= 4,53, 95% CI: 1,25- 16,39; p=0,021) were unfavorable factors. No significant factors were found for LRFS.
Conclusions: The LOT module of the CK-M6 Xsight lung tracking system enables the application of fiducial-free motion management strategies. The advantage of our study is that the most appropriate tracking modality can be selected prospectively before treatment. In our study, excellent local control and a mean survival of 48 months were achieved in patients with NSCLC.
