Acute Rejection in Pediatric Renal Transplantation: Diagnosis of Iatrogenic Obstruction with Tc99m-MAG3 Scintigraphy

Abstract

In renal transplantation, particularly in intraperitoneal grafts performed during childhood, graft and ureteral anatomy may differ from the typical pelvic location seen in adult recipients as a result of somatic growth. This case report aims to emphasize the pivotal and potentially life-saving role of Tc-99m MAG3 dynamic renal scintigraphy in the prompt detection of a surgical complication (ureteral ligation/injury) in a patient with a stable renal graft for 14 years who developed an abrupt rise in serum creatinine and acute renal dysfunction within three days after gynecologic surgery.

Renal transplantasyonlarda, özellikle pediatrik dönemde yapılan intraperitoneal yerleşimli nakillerde, büyüme süreciyle birlikte greft ve üreter anatomisi yetişkinlerdeki pelvik yerleşimden farklılık gösterir. Bu olgu sunumunda; 14 yıldır stabil seyreden bir greftin, jinekolojik cerrahi sonrası üç gün içinde gelişen ani kreatinin yüksekliği ve renal yetmezlik tablosunda Tc99m-MAG3 dinamik renal sintigrafisinin cerrahi komplikasyonu (üreteral ligasyon/yaralanma) saptamadaki belirleyici ve hayat kurtarıcı rolünün vurgulanması amaçlanmıştır.
