Özet
This article aims to evaluate the ethical problems that physicians in specialty training encounter during their education within the context in which they arise, together with their possible causes, and thereby to offer a comprehensive framework for discussion. The article first assesses the ethical problems residents may face-violations and dilemmas-and the attitudes they are able to adopt in response. It then reviews the contextual factors that may influence these problems under the headings of the social environment in which physicians are raised and undergraduate medical education, the determination of the specialty field, the quality of training, and working conditions; the observations and recommendations of the article titled “Medical Ethics in Residency Training”, published approximately 20 years ago, are taken into account for comparison. The article argues that determining the numbers of physicians and specialists according to market demands and political interests, the shaping of medical education by the central examination and “TUS cram schools”, the problems of training environments, and harsh working conditions give rise to significant value-related problems that may be reflected in residents’ training and in service delivery. It concludes that individual counselling or abstract advice will not suffice in the face of this context, and that a collective effort is needed to comprehend the problems together with their causes, develop proposals, and gain a voice in decision-making and oversight mechanisms.
